About IEHP Foundation
IEHP Foundation & History
IEHP Foundation was established in 2021 by Inland Empire Health Plan (IEHP) to ensure all individuals in the Inland Empire have access to Vibrant Health — extending beyond traditional healthcare services and addressing the social determinants of health for the region’s most vulnerable populations.
While IEHP works directly with providers, hospitals and CBOs to provide members with care and direct services, IEHP Foundation works with community leaders and nonprofits to advance health equity for all residents of the Inland Empire – regardless of Medi-Cal membership.

In July 2022, the inaugural IEHP Foundation Board of Directors was established with eight outstanding community leaders and public health experts from San Bernardino and Riverside counties, representing all of the subregions, including the High Desert and Coachella Valley. During the development of the Foundation, the board of directors and executive leadership held dozens of community listening sessions throughout the region with hundreds of nonprofit and community leaders, which informed many of our current strategic initiatives and funding priorities. In February 2024, IEHP Foundation welcomed veteran Inland Empire nonprofit leader Greg Bradbard as its Chief Executive Officer. That same year, the Foundation completed its inaugural strategic plan and established its funding and strategic priorities, all to ensure we fulfill our vision that Vibrant Health exists for all residents in the Inland Empire.

Investing in the Social Drivers of Health Today to Improve Health Outcomes Tomorrow
IEHP Foundation is dedicated to investing in upstream strategies intended to increase the health of residents in our communities while reducing the prevalence of chronic health conditions. By investing in organizations that address social drivers of health – the non-medical factors that account for 60-80% of one’s health outcomes – we aim to accomplish the following:
- Enhance Health Access and Health Equity as residents have increased access to basic needs that advance their ability to take advantage of healthcare services.
- Reduce occurrence & improve management of Chronic Health Conditions, resulting in decreased medical interventions, as residents live in enhanced environments and have increased access to community resources that encourage healthier choices, including diet, physical activity, social connection and reduced risky behaviors.
- Improve HEDIS-related conditions, including high blood pressure, diabetes, maternal and child health, ADHD, mental health and substance use disorders.

Inland Empire at a Glance
The Inland Empire is a vibrant and rapidly growing region, home to over 4.6 million residents – spanning across San Bernardino and Riverside Counties. With its geographic and ethnic diversity, the region represents the future of California as one of the fastest-growing areas in the state, with over 50% of its population being Hispanic/Latino. The IE is known as a region of opportunity, natural beauty and a favorable quality of life.
However, despite its many strengths, the region faces challenges in promoting positive health outcomes for all its residents. While there is room for improvement, the region is collectively working to address these issues and to create a healthier, more inclusive community. By focusing on building strong systems, supporting historically marginalized individuals and families, and empowering communities from within, the Inland Empire is becoming a beacon of Vibrant Health.
San Bernardino and Riverside counties rank below the 50th percentile in the Healthy Places Index (HPI) rankings, which measure health equity across the state. San Bernardino County’s collective average is in the 25th percentile, while Riverside County’s collective average is at the 41.1th percentile (CA Healthy Places Index). By working collectively as a region, we aim to improve these rankings over the coming years.
